This Privacy Policy describes how Seattle Children’s Hospital (“Seattle Children’s”) and its Families Moving Forward Program collect and use data about you through the Families Moving Forward Website (“FMF Website”), including all website pages whose URL includes “familiesmovingforwardprogram.org,” or “familiesmovingforwardprogram.com,” and through other electronic communications between you and Seattle Children’s relating to your use of the FMF Website.
Seattle Children’s is committed to providing its users with an Internet experience that respects and protects their personal privacy choices and concerns. The following discloses our information gathering and dissemination practices for the FMF Website. The FMF Website contains hyperlinks to other websites. Seattle Children’s is not responsible for the privacy practices or the content of such websites.
Web Traffic Data Collected
We may use third party service providers to make the FMF Website available to you and may collect certain web traffic data necessary for the FMF Website to function which may include IP addresses of browsers that visit the FMF Website and other usage data. These data may be used to diagnose technical issues and maintain site security and will not contain your name or email address.
Other Information We Collect and How We Use It
Our website uses a registration form for health care providers trained or seeking training to access training materials and practice resources. We collect a registrant’s organization, name, phone number, email address, recovery email address, username and password.
We use the collected information to verify that the user is the same individual who has applied for and paid for or completed training provided by Seattle Children’s or an affiliated organization providing training. The information is used to provide access to proprietary training materials and materials for health care practitioners to use once training is completed. A user’s access is revoked if they do not complete training.
We may also use the information described above to periodically contact practitioners who have completed training to: (1) verify and update contact information; (2) gather a report of program and resource use; (3) provide program updates; and (4) determine practitioner preferences for sharing their information with new program partners. You may request to be removed from email lists by responding to any individual message or by emailing fa*******************@se**************.org .
If a user so requests, we may use the user’s contact information to send the user additional information about Seattle Children’s, the Families Moving Forward Program, or program affiliates.
Disclosure of Your Information
We disclose a user’s information, including name and email address, with program partners and affiliates who process training applications and provide program training, including the Florida Center Training Institute (FCTI), to verify a user’s application and registration for training. We disclose name and email address of past trainees with FCTI to enable FCTI to contact past trainees with information about additional relevant trainings offered by FCTI. If a user requests, we may disclose the name and email address of the user to affiliates, partners, contractors, agents, or other appropriate parties as necessary to satisfy the user’s request for more information and resources.
With a user’s express permission, we may disclose their name and email address to FASD United for the purpose of placing the user on an FASD United directory of resources for families affected by FASD/PAE. The FMF Website will link to this FASD United directory.
The information we collect is not used for any purposes other than those described above and will not be disclosed to anyone other than Seattle Children’s and its affiliates, partners, contractors, and agents, unless necessary to comply with a court order or other legal process, to protect the rights or property interests of Seattle Children’s, or at the user’s request.
The FMF Website, including the FMF Client Portal, is not intended for storage of any identifiable patient information, including information protected under the U.S. Health Insurance Portability and Accountability Act (“HIPAA”) and/or applicable state privacy laws. Users are prohibited from posting or entering such information to the FMF Website, including in the FMF Client Portal, or transmitting such information to Seattle Children’s in electronic communications related to the FMF Website.
Security
This website has security measures in place to protect against the loss, misuse and alteration of the information under our control. Information is stored on servers. Access to change or edit content/information is restricted to those authorized by Seattle Children’s to do so.
The safety and security of your information also depends on you. You have chosen a username and password for the use of the FMF Website; you are responsible for keeping your username and password confidential. Please do not share your username and password with anyone.
Unfortunately, the transmission of information via the Internet and through electronic communications is not completely secure. Although we take appropriate precautions to try and protect your data, we cannot guarantee the security of your data transmitted to the FMF Website or in electronic communications about your use of the FMF Website. Any transmission of data is at your own risk. We are not responsible for circumvention of any privacy settings or security measures contained on the FMF Website or in your operating system.
Updates and Changes to Privacy Policy
We reserve the right, at any time, to add to, change, update or modify this Privacy Policy, simply by posting such change, update, or modification on the FMF Website. Any such change, update, or modification will be effective immediately upon posting.
